However, not all lower-limb wounds are venous in origin: before starting compression therapy or a procedure, the cause must be clarified, and arterial circulation, general health and any factors preventing healing must also be assessed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">At Centro Medico Gargano in Naples, the pathway includes clinical assessment, examination of the wound and, when indicated, <a href=\"https:\/\/centromedicogargano.com\/en\/venous-colour-duplex-ultrasound-naples\/\">venous colour Doppler ultrasound<\/a>. Treatment is chosen according to the cause, the presence of oedema and reflux, the skin condition and tolerance of compression.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">How to recognise a possible venous ulcer<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The appearance of a wound alone does not always allow a definite diagnosis. Findings consistent with a venous origin may include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>a location on the lower part of the leg, often near the ankle;<\/li>\n\n\n\n<li>oedema and a feeling of heaviness;<\/li>\n\n\n\n<li>varicose veins or previous episodes of venous thrombosis;<\/li>\n\n\n\n<li>brownish pigmentation, eczema, hardening or other skin changes;<\/li>\n\n\n\n<li>a wound that tends to persist or recur.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Significant pain, a cold or pale foot, reduced peripheral pulses, necrosis, fever, rapidly spreading redness, a marked foul smell or sudden deterioration require prompt assessment. Results are not identical for every patient.<\/figcaption><\/figure>\n<\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">Why a complete diagnosis is needed<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Initial assessment considers wound duration, previous ulcers, thrombosis, venous procedures, pain, oedema, medicines, diabetes, mobility and other relevant conditions. Clinical examination includes the skin, lesion, peripheral pulses and signs of venous or arterial insufficiency.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Before applying significant compression, adequate arterial circulation must be confirmed through clinical assessment and appropriate investigations. When a venous component is suspected, colour Doppler ultrasound examines the superficial and deep systems, looks for reflux or obstruction and helps establish whether treatment of a responsible vein is indicated.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Components of treatment<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">Compression, when appropriate<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Compression therapy is central to managing uncomplicated venous ulcers, but it must be prescribed and applied appropriately. Bandage or stocking type, compression strength, leg shape, oedema, mobility and tolerance must be assessed individually. Arterial disease, neuropathy, heart failure or other conditions may require adjustments or specialist supervision.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Wound care<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The wound is cleaned and protected with a dressing suited to the amount of exudate, surrounding skin and patient comfort. Debridement may be indicated where non-viable tissue is present, but technique and timing depend on the findings. No dressing replaces control of venous hypertension when that is the main cause.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Infection and antibiotics<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Bacterial colonisation of a chronic wound does not automatically mean infection. Systemic antibiotics are considered when clinical signs are consistent with infection and should not be used routinely simply because an ulcer is open.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Treating venous reflux<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">If colour Doppler ultrasound documents significant superficial venous reflux and the patient is suitable, early treatment of the responsible vein, combined with compression and wound care, may support faster healing and increase ulcer-free time. The technique depends on anatomy and may include thermal endovenous procedures, <a href=\"https:\/\/centromedicogargano.com\/en\/sclerotherapy-varicose-thread-veins-naples\/\">ultrasound-guided sclerotherapy<\/a> or other options in selected cases.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Sclerotherapy does not automatically replace every other procedure and is not indicated for all ulcers. It may help when reflux or veins around the ulcer are accessible to this technique, after assessing the deep system, arterial circulation, thrombotic risk and general health.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The pathway at Centro Medico Gargano<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Assessment is overseen by Dr Ciro Gargano, a medical doctor and specialist in Diagnostic Radiology practising angiology and phlebology. Ulcer size and duration, oedema, arterial disease, mobility, diabetes, infection, adherence to compression and reflux characteristics influence the response. Recurrence can occur even after the wound closes, so maintenance compression when indicated, follow-up and treatment of correctable venous factors remain important.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Frequently asked questions<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">Is every leg ulcer venous?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">No. Ulcers may be arterial, neuropathic, inflammatory, traumatic, neoplastic or mixed. A wound that does not heal needs assessment, not just a dressing.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Can compression be applied straight away?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Not always. Arterial circulation and any contraindications or precautions must be assessed first. The professional should select compression strength and system according to the patient.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Does sclerotherapy heal all venous ulcers?<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">No. It can treat certain reflux patterns or varicose veins in selected patients but must form part of a programme including diagnosis, compression, wound care and follow-up.
