Swollen and heavy legs do not automatically mean venous insufficiency. The problem may be related to posture, heat, varicose veins, lymphoedema, medicines, excess weight, joint problems or general health conditions. The distribution of swelling, how quickly it develops and associated symptoms help distinguish a situation suitable for a scheduled assessment from a possible emergency.
At Centro Medico Gargano in Naples, the consultation may be supplemented by venous colour Doppler ultrasound when the clinical question concerns thrombosis, reflux, varicose veins or other venous system abnormalities.
Possible causes
- prolonged immobility, long periods of standing or high temperatures;
- venous insufficiency, varicose veins or consequences of thrombosis;
- lymphoedema, lipoedema with an oedematous component or venolymphatic oedema;
- medicines, obesity, reduced mobility or joint problems;
- heart, kidney, liver, endocrine, inflammatory or infectious conditions.
Swelling in both legs that gradually increases towards evening differs from one leg suddenly becoming larger, warm and painful. It is therefore inappropriate to choose stockings or treatment before at least clarifying the most likely causes.
Signs that require attention
- sudden swelling of one leg, calf pain, redness or warmth;
- breathing difficulties, chest pain, fainting or a rapid heartbeat;
- fever, very red skin or suspected infection;
- an ulcer, bleeding from a varicose vein or rapidly worsening skin;
- persistent oedema with breathlessness, reduced urine output or rapid weight gain.
If you have breathing difficulties or chest pain, it is not appropriate to wait for a routine outpatient consultation.
When the cause may be venous or lymphatic
A venous cause is more likely when symptoms worsen after many hours of standing or sitting and occur with varicose veins, heaviness, itching, cramps, pigmentation or previous thrombosis. Lymphoedema may be asymmetrical, involve the foot and become firmer over time; lipoedema typically involves a symmetrical, painful increase in fatty tissue, easy bruising and sparing of the feet.

What colour Doppler ultrasound is used for
The examination assesses the deep and superficial veins and may look for thrombosis, document reflux, assess the effects of previous thrombosis or treatment, and help plan a procedure. It does not independently diagnose lymphoedema, lipoedema, fluid retention or cardiac, renal and hepatic causes. A normal venous examination may therefore mean that further investigations are needed.
Assessment and treatment
The consultation considers the pattern and symmetry of swelling, pain, medicines, known illnesses, previous thrombosis or procedures, and weight changes. Depending on the findings, colour Doppler ultrasound, limb measurements, segmental bioimpedance analysis, blood tests or assessment by other specialties may be indicated.
Treatment depends on the cause. It may include movement, breaks from prolonged immobility, weight management, care for the underlying condition and correctly prescribed compression. Where reflux and varicose veins are present, options may include sclerotherapy, saphenous vein treatment or other procedures. In lymphoedema, the programme instead centres on compression, movement, skin care and decongestive therapy.

Are swollen legs always caused by circulation problems?
No. There are also lymphatic, medication-related, joint-related and systemic causes.
Is colour Doppler ultrasound always needed?
No. It is indicated when the consultation raises a question about the venous system.
Are compression stockings always helpful?
No. The type, size and class must suit the diagnosis, the shape of the limb and the arterial circulation.
